One insidious way this happens is cutting and pasting 
text into the email client which may inadvertently cause
the message to use a non plain text encoding. This all depends
on the application you cut and paste from, but it had caught me
out in the past.

Obvious client side effects are fancy fonts and font control
bold, italic, underline etc.

Looking at the archives the recent MIME crimminals are
fairly obvious as that is only plain text and a good way to
check if you are conforming.
