And increase the malpractice insurance by an order of magnitude.  Ends
up a zero-sum or losing proposition.

1.  A major or colonel makes a LOT (LOT LOT LOT) less than a family GP
in the US, let alone a specialist.

2.  The doctors, nurses, etc. for what you call "urgent in theater
cases" are A) still on payroll and B) have to keep up their skills
doing something.
