I believe it is supposed to look more like this:

     damage boxes   Threshhold roll:
(12) XXXXXXXXXXXX	     6
(10) XXXXXXXXXX 	     6
 (8) XXXXXXXX		     6
 (6) XXXXXX		     6
 (4) XXXX		     6
 (2) XX 		  dead
