The problems with formations in my experience are:

(a) you can't usefully mix thrust levels within a formation. Not a
problem if you're operating multiple ships of the same class, but it can
be somewhat limiting.

(b) when ships start to take drive thresholds, they have to fall out of
formation; this increases the number of effective manoeuvre units you
have, just when the game's already slowing down because you have to make
lots of decisions...
