I was thinking that maybe they should have a multiplier, similar to
heavy
weapons fire vs. armored vehicles. For example:

Grenade impact vs. a figure in hard cover: d12 / 2
Grenade impact vs. a figure in soft cover: d12
Grenade impact vs. a figure in the open: d12 x 2
Grenade impact vs. a figure in a building: d12 x 3
